Letter of Agency or Authorization (LOA)

A Letter of Agency or Authorization (LOA) is the form that gives AVATAVA the authorization (or to act as your agent) to port the number(s) from the losing carrier to you.  Please make sure to complete the form in detail as incomplete forms will not allow AVATAVA to complete the port.
